Life Insurance
Life insurance gives you peace of mind knowing that your loved ones will be financially protected if something happens to you. The County of Summit provides Basic Life and Accidental Death & Dismemberment (AD&D) insurance at no cost to most full-time employees through Guardian Life. Additionally, employees have the option to purchase Voluntary Life and AD&D coverage through Guardian Life to supplement their basic coverage and provide an added layer of financial protection.
Basic Life and AD&D Insurance (County Provided)
The County of Summit provides Basic Life and Accidental Death & Dismemberment (AD&D) insurance for most full-time employees at no cost. The amount of your Basic Life insurance policy is determined by your bargaining unit status. Please refer to the benefit guide.
- Basic Life Insurance pays a benefit to your designated beneficiary if you die while coverage is in effect
- AD&D Insurance provides additional financial protection in the event of a covered accidental death or serious injury, such as the loss of a limb, sight, or hearing
Voluntary Life and AD&D Insurance (Employee Purchases)
Employees may purchase additional Voluntary Life and AD&D coverage for themselves or their spouse in amounts ranging from $10,000 to $300,000, increments of $10,000, through payroll deductions. Evidence of Insurability (EOI) may be required and must be completed online through Guardian's website. Please note: Lifetime Life Insurance with a Long-Term Care Feature is also available through Voya Financial. Beneficiary Designation
A beneficiary is the person or persons you designate to receive your life insurance benefit. It is important to designate both a primary and secondary (contingent) beneficiary to ensure your benefits are distributed according to your wishes. The beneficiary designations must add up to 100%.
- Primary Beneficiary — the first person(s) to receive the benefit
- Secondary (Contingent) Beneficiary — receives the benefit if the primary beneficiary is unable to do so
- The beneficiary designations must add up to 100%.
Beneficiaries may be updated during the Annual Open Enrollment Period or at any time during the plan year by completing the beneficiary form(s) posted below. If you have both Basic Life and Voluntary Life coverage, please complete both beneficiary forms when making changes. Completed forms may be emailed to the Division of Employee Benefits, insurance@summitoh.net.
Portability and Conversion
If your employment ends, you may have the option to port or convert your life insurance coverage. For assistance with conversions, portability or disability benefits, employees may contact Guardian’s National Conversion Department directly at national_conversions@glic.com or (800) 433-5982.
