What do I need to do if I have a Qualifying Life Event mid-year?

You have 30 days from the date of the event to submit the appropriate paperwork (court documents, birth certificates, etc.) to the Division of Employee Benefits at insurance@summitoh.net and complete the online process. At that time, you may change benefits, enroll in benefits or cancel benefits.
